This form allows Dr Rinky Vamvadelis to review your travel plans, medical history, and vaccination record in advance — ensuring your consultation is efficient and your advice is tailored to your destination and personal health needs.

Please email the completed form to info@thehfp.co.za or bring a printed copy to your appointment. New patients should also complete the New Patient Registration Form before visiting the practice.

It’s best to schedule your vaccination appointment 4 to 6 weeks before you travel. Some vaccines require multiple doses or take up to two weeks to become fully effective.

Vaccine requirements differ depending on where you’re going — for example, Africa, Asia, South America, or the Indian Ocean Islands may require different immunisations. Dr Vamvadelis will advise you according to the latest South African and international travel guidelines.
